YOUR FIRST VISIT
For your first visit, please arrive at least 15 minutes early and check in with our receptionist. Bring insurance information with you and loose-fitting comfortable attire to wear during your evaluation (T-shirts, shorts, athletic shoes, etc.).

- All Private Commercial Non-HMO Healthcare Plans
- Workers' Compensation
- No Fault
We check with out of area plans prior to patient appointments to obtain coverage details .
Insurance information changes periodically so please contact our office if you do not see your insurance carrier listed above.
We are also accepted by many other insurance companies through out-of-network benefits. Most Major Insurance carriers cover all or part of the fees associated with care provided by a licensed physical therapist. You should verify your policy's coverage of Physical Therapy services prior to your initial evaluation.
SELF PAYMENT OPTIONS
Prescription Fitness Physical Therapy also offers private pay options to our patients. Patients can select to pay on a per-visit basis or purchase a cost-saving prepaid package. Please contact us for more details and current pricing.
